Wellness Screening Questionnaire

We look forward to welcoming you to Mohonk Mountain House! In order to keep our mountaintop safe, we are requiring all arriving guests complete a health screening questionnaire and sign our assumption of risk prior to arrival. All guests staying at Mohonk will be required to have this health form on file prior to entering property. If your reservations include other guests who will not be traveling with you, please ensure each of them completes this prior to arrival by forwarding this email to them.
 
In order to expedite your arrival through the gatehouse, we highly recommend completing a waiver for each person in your travel party by midnight the day prior to your arrival. This waiver cannot be filled out more than 48 hours in advance to your arrival.
